The Care Coordinator serves as the primary point of contact between clients, families, caregivers, and the internal office team. This role is strongly focused on proactive communication, quality assurance, and maintaining accurate and up-to-date care plans.
Responsibilities:
• Make daily outbound calls to clients and caregivers for quality checks, feedback collection, and proactive issue resolution.
• Maintain constant communication with caregivers regarding schedules, expectations, updates, and care requirements.
• Update and maintain accurate and detailed client care plans, ensuring they reflect current needs and services (no incorrect or outdated tasks).
• Clearly document all interactions, updates, and incidents in CRM systems.
• Coordinate between clients, caregivers, and the internal team to ensure smooth communication and efficient service delivery.
• Monitor trends in care quality, identify recurring issues, and support continuous improvement initiatives.
• Audit caregiver call logs and ensure compliance with documentation standards.
• Assist with basic administrative and insurance-related tasks.
• Provide real-time updates to the office team to facilitate in-person interventions when needed.
Requirements:
• English level: C1
• **More than 1 year of experience in home care, healthcare coordination, or a related field
• Strong knowledge of home care workflows and terminology
• Experience managing communications with both clients and caregivers
• Experience with CRM systems or care management platforms (OneSky, WellSky, or similar) is preferred
• Ability to handle sensitive situations with empathy and professionalism
