The Resident Care Coordinator (RCC) at Savoy Life, a subsidiary of Red Cell Partners, plays a pivotal role in delivering healthcare services to seniors in Chubbuck and Pocatello, Idaho. This position involves close collaboration with a dedicated RN and provider to offer comprehensive care in various settings, including assisted living, independent living, memory care units, and individual homes. Key responsibilities include scheduling patient visits in the electronic health record (EHR) system to optimize the time of RNs and Advanced Practice Providers (APPs), coordinating with patients and caregivers to meet their needs. The RCC is also responsible for managing communications, such as answering calls, texts, and voicemails professionally and promptly, as well as handling inbound and outbound faxes. Maintaining accurate and organized patient records by entering data, scanning, and printing files is essential. Additionally, the RCC identifies and manages health plan insurance for residents, supports telehealth visits, and fosters positive relationships with patients, caregivers, families, and other healthcare providers.

Candidates must possess a high school diploma or GED, with at least two years of experience in healthcare focusing on care coordination and administrative support. While certification as a Medical Assistant or Patient Care Technician is preferred, individuals with five or more years of relevant healthcare experience will also be considered. Experience in geriatric care or within senior living communities is highly desirable. A valid driver's license is required, as the role involves traveling to various senior living settings and individual homes. Proficiency in electronic medical record documentation and comfort with various technologies are essential.
